Exhibit A <br />page 8 of 8 <br />• Traffic Rodeo Certification Forms <br />Each school contact will be given a Certification Form to verify the rodeo <br />event conducted (i.e. date, time, number of participants). In addition, the <br />school administrator will be asked to complete the survey portion of the <br />Certification Form. The survey portion of the form will include questions on <br />program content, presentation, printed material and effectiveness of the <br />program. The form also allows for additional comments and/or suggestions. <br />All surveys will be reviewed and evaluated after each rodeo. <br />• Transportation Surveys & Statistics on Death and Injury <br />Safe Moves will conduct transportation surveys at each participating school <br />and senior citizen center to assess the modes of transportation used by <br />students and seniors. In addition, Safe Moves will establish statistical data on <br />traffic -related deaths and injuries among the target group for 1997-98. This <br />data will be compared to the base line data for 1996-97 for evaluation of the <br />effectiveness of the program. <br />• Monthly and Quarterly Program Reports <br />Safe Moves will prepare and submit Monthly Program Reports to the City of <br />San Leandro. These reports will include all scheduled workshops and rodeos <br />with dates, locations, contact names, addresses and telephone numbers. Also <br />to be included are certification reports, schedule of upcoming programs, <br />accident data/ copies of support letters and billing documentation. <br />• Final Report <br />Safe Moves will submit a final report to the City of San Leandro for review <br />within ten (10) days of the completion of the program. <br />Total Program Costs: $ 20,800.00 <br />Page 8 of 8 <br />
