The participation of the Transportation Department and the <br />Police Department will be based on the availability of personnel. <br />Safe Moves will promote the presentations with flyers and posters distributed <br />to churches, senior citizen agencies, health care providers community centers, <br />libraries and retail establishments. <br />The City of San Leandro will provide Safe Moves with a list of all city <br />agencies that provide services to senior citizens that are to be included in the <br />program. <br />Fee per presentation: $ 650.00 <br />Total Fee: $ 1,950.00 <br />4. Method of Evaluation <br />In order to evaluate the effectiveness of the program, the following <br />procedures will be incorporated: <br />• School Workshop Certification Forms <br />Each school administrator will be given a Certification Form to verify <br />workshops conducted (i.e. date, time, number of students and number of <br />workshops conducted). In addition to the program data, each school <br />administrator is required to complete the form which provides feedback on <br />the program content, presentation delivery, printed material and <br />effectiveness. The form also allows for additional comments and/or <br />suggestions. <br />Page 7 of 8 <br />
