County shall use the annual program objectives as the basis of measuring performance. To document <br />program progress, quarterly reports shall be submitted to City and shall include the following information: <br />1. Name and address of applicants <br />2. Date of call for service <br />3. Date of site visit <br />4. Dates of start and completion <br />5. Description of work performed <br />6. Costs of work performed <br />7. Name of contractor <br />8. Income and ethnicity of clients <br />9. Total number of applications <br />10. Total number of units under construction <br />11. Total number of units completed or committed <br />12. Total costs of projects, including administrative costs, quarterly and year-to-date. <br />IX. DOCUMENTATION AND RECORD -KEEPING <br />County shall maintain all records required by the federal regulations specified in 24 CFR Part 570.506, and that <br />are pertinent to the activities funded under this agreement. Such records shall be preserved for a minimum of <br />three years following termination of all activities funded under this agreement, or after the resolution of all <br />Federal audit findings, whichever occurs later, and shall include but are not limited to: <br />1. Documentation of the eligibility of program recipients <br />2. Documentation of compliance with fair housing and equal opportunity policies <br />3, Financial records required by 24 CFR Part 570.502 and OMB Circular A-110; and <br />4. Other records to document compliance with Subpart K of 24 CFR 570. <br />City shall have the right to inspect records pertaining to this agreement and activities undertaken by County <br />pursuant to this Agreement, Such inspections shall occur during normal business hours. <br />X. MARKETING ASSISTANCE <br />County shall market the program to organizations representing target populations, in particular seniors and the <br />disabled, establishing information booths at neighborhood fairs and placing notices in neighborhood <br />newsletters. Mailings shall be conducted on a quarterly basis in City's targeted area outlining program <br />services. Public Service announcements and advertisements in other major media are planned. Presentations <br />will be conducted at senior centers and to other locations, as often as necessary, to inform the public of the <br />range of services and programs available. <br />EXHIBIT A Page 3 of 4 <br />HCD Agreement July 2002 <br />
