7. Program Monitoring and Reporting <br />Contractor shall submit written semi-annual reports to the City. The semi-annual reports <br />shall include, but not be limited to, the following data elements, in the format contained in <br />the reporting forms, Attachment 1. <br />A. Title of program, listing of components, description of activities/operations. <br />B. Service areas (i.e., citywide, etc., including applicable census tracts). <br />C. Goals —The projected goals, indicated numerically, and also the goals achieved <br />(for each report period). In addition, identify by percentage and description, the <br />progress achieved towards meeting the specified goals; additionally identify any <br />problems encountered in meeting goals <br />D. Beneficiaries — Provide the following in the forms provided under Attachment 1: <br />1) Total number of direct beneficiaries. <br />2) Income level of beneficiaries, by income bracket, number of beneficiaries <br />who are extremely low-income, very low-income, and low-income. <br />3) List of all beneficiaries by ethnicity as shown in ethnicity table. <br />4) Number of beneficiaries who are part of a female headed household. <br />5) Number of beneficiaries who are disabled. <br />6) Number of beneficiaries who are seniors (62 or older). <br />7) Number of beneficiaries who are part of a large household (5 + persons). <br />Exhibit A— Building Futures page 2 of 2 <br />September 16, 2002 <br />GACommunity Development\CDBG\BuildingFutures\FY2002-03 <br />
