19 <br />SLPOA MOU January 1, 2023 – June 30, 2028 <br />Section 23. Resignation <br /> <br />An employee wishing to leave City employment in good standing shall file with the supervisor at <br />least two (2) weeks before leaving the City service a written resignation stating the effective date <br />of resignation and reasons for leaving. <br /> <br />The resignation shall be forwarded to Human Resources with a statement by the Police Chief as <br />to the resigned employee's service performance and other pertinent information concerning the <br />cause for resignation. <br /> <br />Failure of an employee to comply with this rule shall be entered on the service record of the <br />employee and may be cause for denying future employment by the City. The resignation of an <br />employee who fails to give notice shall be reported to Human Resources immediately. <br /> <br />Section 24. Employee Benefits <br /> <br />24.1 The IRS 125 plan shall continue. The plan shall provide the following: <br /> <br />A. Pretax conversion of employee contribution toward medical premiums. <br /> <br />B. Medical Flexible Spending Account with a maximum employee pretax contribution as <br />determine by the IRS for the benefit plan year. <br /> <br />C. Dependent Care Flexible Spending Account with a maximum employee pretax <br />contribution as determine by the IRS for the benefit plan year. <br /> <br /> The City will maintain a “core flex” medical benefit plan. The core shall consist of the <br />CalPERS Medical plan and a dental plan. <br /> <br />24.2 The City will contribute the monthly amounts below, which includes the CalPERS Medical <br />Plan “Minimum Employer Contribution” towards the election of medical and dental benefits <br />in the plan, or the actual premiums, whichever is less. As of January 1, 2023, these monthly <br />contribution amounts are: <br /> <br />Coverage Level City Pays <br /> <br />Medical <br />Employee only $ 870.17 <br />Employee + 1 $1,740.33 <br />Employee + 2 or more $2,262.42 <br /> <br />Dental <br />Employee only $ 49.83 <br />Employee +1 $ 94.60 <br />Employee +2 or more $147.38 <br /> <br />In January of each year, the City will pay 70% of the increase in medical and dental <br />premiums based on Kaiser and the dental core plans (e.g., 2024 premiums minus 2023 <br />premiums, multiplied by 70%). <br /> <br />Employees electing not to enroll in the core flex medical plan (i.e., who wish to waive <br />enrollment in the medical and dental plans) and demonstrate to the satisfaction of the City
