<br />Consulting Services Agreement between City of San Leandro and Last revised 6/16/2023 <br /> EBAC for CAP Grant: Family Resource Center Linkages Exhibit A – Page 1 of 3 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br /> <br />The program model aims to benefit families by providing comprehensive services to address health <br />disparities, strengthen resilience, and support economic stability and housing security. The services offered <br />include prevention, response, and reduction of health disparities, increased access to food resources and <br />distribution programs, resource navigation and support for economic stability, mental health and wellness <br />support for parents/caregivers, and collaboration with school district Coor dination of Services Teams <br />(COST). <br />The Family Resource Specialist (FRS) plays a crucial role in directly assisting families to achieve economic <br />stability and food security. The FRS provides case management services, outreach, and problem -solving to <br />help families access various services such as domestic violence support, housing assistance, food <br />programs, employment resources, and behavioral health services. <br />EBAC's San Leandro Family Resource Center (FRC) acts as a hub for accessing services that promote <br />economic stability, family wellness and resiliency, and violence prevention. The center offers service <br />navigation, assistance with health coverage and food stamps enrollment, parent education and support <br />activities, as well as concrete support like providing food and clothing. <br />EBAC's services include directly assisting families in accessing services from both EBAC and other <br />providers, supporting families with potential mental health needs through school COST meetings, offering <br />education and support to help families find and retain permanent housing, providing housing information <br />and support for unhoused families, assisting families in enrolling into food assistance programs, helping <br />with unemployment applications, making referrals to promote economic and housin g stability, and <br />maintaining relationships with district Parent Liaisons to reach vulnerable families. <br />EBAC shall perform Medi-Cal Administrative Activities (MAA) to assist in the proper and efficient <br />administration of the Medi-Cal Program by improving the availability and accessibility of Medi-Cal Services <br />to Medi-Cal eligible and potentially eligible individuals and their families. These activities include: Medi -Cal <br />Outreach, Facilitating Medi-Cal Application, Medi-Cal Non-Emergency Transportation, Contracting for <br />Medi-Cal Services, Program Planning and Policy Development, Medi -Cal Administrative Activities <br />Coordination and Claims Administration and Training. <br /> <br />This includes but is not limited to: <br />• To ensure that MediCal potentially eligible individuals, and their families where appropriate, served <br />by the Contractor are informed of the Medi-Cal Program, how to access it, and assisted in <br />accessing the MediCal Program, if needed. <br />• To ensure that assistance is provided to MediCal eligible individuals, and their families where <br />appropriate, in facilitating their receipt of services and activities in the MediCal Program. <br />• Ensure all applicable County and State policies and procedures pertaining to MAA, as contained in <br />memos, training materials, updates, and other written communications with MAA Contractors, are <br />met. <br /> <br />DocuSign Envelope ID: B16ECB49-B03A-4724-9CCF-59EB56448DCA
