<br />Consulting Services Agreement between City of San Leandro and Last revised 6/16/2023 <br /> EBAC for CAP Grant: Family Resource Center Linkages Exhibit A – Page 2 of 3 <br /> <br />Goal: <br />The goal of the program is to improve the health, wellness, and resiliency of San Leandro residents facing <br />significant risk factors for poor health outcomes and family violence. This will be achieved through family <br />wellness navigation support services, grief/loss support, and parenting education that reduce stressors, <br />improve family functioning, and build protective factors. <br /> <br />Objectives: <br />1. Provide navigation support to 30 unduplicate d families. <br />2. Reach 20 families (not unduplicated) through the Ready4K program. <br />3. Conduct 2 grief/wellness workshops or groups (one per semester). <br />4. Allocate 3-5 hours per week for walk -in, drop-in, and other brief services to non-client service recipients. <br />5. Participate in 5 school district COST team meetings. <br /> <br />Outcomes: <br />The program aims to achieve the following outcomes: <br />1. 80% satisfaction with services. <br />2. 85% increase in understanding of community resources. <br />3. 85% of participants report feeling be tter prepared to seek support when needed. <br />4. 23 out of 30 case management families (75%) will meet at least one family goal based on the protective <br />factors framework. <br /> <br />Service Components: <br />1. Service Navigation and Resource Linkage: Increase access to medical, dental, mental health, food, <br />employment, and housing services. <br />2. Family Wellness/Parenting Workshops: Support increased social connections and support, particularly <br />related to grief/loss. <br />3. Ready4K Parenting/Child Development Texts: Provide texts in multiple languages to enhance child <br />learning. <br />4.
