<br />Consulting Services Agreement between City of San Leandro and Last revised 6/16/2023 <br /> EBAC for CAP Grant: Family Resource Center Linkages Exhibit A – Page 3 of 3 <br /> <br />Project-specific goals: <br />1. Increase food security for San Leandro residents. <br />2. Support economic stability through service navigation, increasing access to assistance services, and <br />promoting protective factors. <br />3. Promote family wellness through support, education, and collaboration. <br /> <br />Impacts: <br />The program seeks to achieve the following impacts: <br />1. Increased utilization of concrete and family support services. <br />2. Enhanced social connections, cohesion, and trust. <br />3. Support for grief/loss. <br />4. Improved access to economic supports. <br />5. Enhanced parental understanding of child development, child-rearing, nurturing/attachment, and positive <br />parenting. <br /> <br />Tools: <br /> <br />EBAC uses an internal assessment tool to gather demographics to better understand client need/service <br />usage. Client level data is entered into our electronic health records (EHR) system. Aggregated <br />demographic/outcome data inform analysis of/appropriate response to racial and other inequities in service <br />delivery. Families receiving at least 3 months of services complete a Protective Factors Survey (PFS) <br />measuring progress and impact.
