Software-As-A-Service (SaaS) Agreement between the City of San Leandro and City Data Services FY23-24 11 of 12 <br />\ <br /> <br /> <br /> <br /> <br /> Housing Division FY 2023-24 <br />City of San Leandro <br />835 East 14th Street San Leandro, CA 94577 <br />City Data Services provides web-based data management services for grants and loans that fund public services, <br />economic development, housing programs and projects. The following summary indicates a general understanding <br />between the City of San Leandro and CDS. It represents a Hybrid Scope of Work for both the continued Set- <br />up/Development in addition to the monthly Maintenance of Services provided by CDS using both the Public Services <br />Module & Housing Module (Multi-Family and elements of the Single-Family Modules). Following is a Scope of Work that <br />will be provided to the for City of San Leandro in FY 23-24 for a monthly fee of $780: <br /> I. SCOPE OF SERVICES –Hybrid Public Services & Housing Module – continued Set-up/Development <br />1) Develop web-based data management module for City of San Leandro to include but not limited to <br />the following features for both Public Services and Housing Project Management: <br />• Ability to enter, store, display, and summarize individual project administration information. <br />• Ability to enter, store, display, and summarize individual project contract information. <br />• Ability to enter, store, display, and summarize individual project report information and, <br />• Ability to transmit project activity information into HUD's reporting system, IDIS, as needed. <br />2) System to generate, display, archives, and print the following documents/forms as needed for either Public Services or Housing Project Management: <br />• Request for Proposal Documents – In FY 23-24: CDBG/CAP RFP for Public Services and CIP funding applications <br />• Project Application and Application Scoring Reports – In FY 23-24: CDBG/CAP RFP for Public Services funding project review & allocation <br />• Application Review process for Staff, Boards and/or Commissions – In FY 23-24: CDBG/CAP RFP for Public Services funding project review & allocation <br />• Loan Amounts and Payment Information – In FY 23-24 complete set-up and review of Multi-Family Asset Management <br />• Scope of Work Documents – In FY 23-24: CDBG/CAP Public Services funding allocations <br />• M a n a g e m e n t & Occupancy Forms – In FY 23-24 complete set-up and review of Multi-Family Asset Management <br />• B i l l i n g F o r m s & Check Request – ongoing for CDBG Public Services Project Management <br />• Compliance & Monitoring Forms – ongoing for CDBG Public Services Project Management and Multi-Family Asset Management <br />• Project Review Forms – TBD when City Affordable Housing Funding becomes available for NOFA Project Management <br />• Various HUD/State forms and/or other forms – ongoing for CDBG Project Management: HEROS (when available), Section 3, and MBE/WBE. <br />• Periodic Reports <br />• Annual Summary Reports <br />• Compliance Summary – ongoing for Multi-Family Asset Management <br />3) Assistance with set-up and workflow: <br />• Provide assistance to set-up and migrate existing spreadsheets - project information into CDS system. Explore using the Multi-Family Asset Management module for Mobile home Space Rent Stabilization Ordinance enforcement including annual rent review and compliance with CPI allowed increases. <br />4) System Timing <br />• Working with Staff, CDS will have system in place within 30 days of receipt of housing data. <br />II. SCOPE OF SERVICES – System Maintenance <br />Under the scope of the maintenance contract, CDS will provide: <br />• Ability to store, display, and summarize individual program administration information, <br />• Ability to store, display, and summarize individual program contract information, <br />• Ability to enter, store, display, and summarize applications, reports and fiscal information to assist with <br />management of Community service activities, <br />• Ability to transmit sub-recipient report information into HUD's reporting system, IDIS, as needed, <br />• Maintain all database and report elements created under this development proposal, <br />• Maintain data backups and download to the City of San Leandro on a determined schedule, <br />Exhibit B <br />DocuSign Envelope ID: 2CAD6DD6-2718-4320-8D8C-86D96A6A60A9
