<br />Consulting Services Agreement between City of San Leandro and <br />Gates & Associates, Inc for Staff Augmentation Exhibit A – Page 24 of 31 <br />○ Respond to emails and phone calls after PG&E designs have been approved and <br />prior to construction start <br />○ Project coordination meetings after PG&E designs have been approved and prior <br />to construction start <br />● Provide Dry Utility Construction Administration <br />○ Respond to RFI’s and submittals <br />○ One jobsite visits <br />○ Respond to emails and phone calls <br />○ Meetings <br />○ Design updates and revisions for as built drawings. Actual locations of trenching <br />and equipment installation to be confirmed and provided by contractor or others. <br />Subtask 7.2 – Construction Observation <br />● Attend (12) twelve construction observation visits throughout the construction phase. <br />Consultants shall issue an observation report after each visit in accordance with the City <br />timelines <br />● Attend walk-through of the Project site, review Contractor Punch List, and provide <br />written response with status and action of items on the Punch List. Consultant shall <br />attend final walk-through of the Project site with the City, verify Punch List completion, <br />and provide written response with recommendation regarding Project acceptance and <br />close-out. <br />Subtask 7.3 – Construction Meetings <br />● Attend weekly OAC meetings for a duration of 12 months. <br />Subtask 7.4 – Post Construction Documentation & Training <br />● Review Contractor-supplied Operation and Maintenance manuals and Warranties to <br />determine their completeness and compliance with Construction Contract and provide <br />written recommendation for acceptance. <br />● Coordinate O&M training <br />● Prepare digital files of Record Drawings that reflect the changes to the work during <br />construction based upon marked up prints, drawings and other data furnished by the <br />Contractor and City in the approved City format. <br />● At ten (10) months following the issuance of final completion Consultant shall visit the <br />site and: 1) Review the work and identify observable defects and deficiencies, 2) Evaluate <br />the performance, durability and appearance of installed products, materials and system <br />as they relate to suitability for the intended use; 3) Evaluate the Project’s function and <br />City’s use of the Project as reflection of the original program intent; and 4) Submit a <br />written memorandum to City concerning the foregoing no later than 305 days after <br />issuance of final completion. <br />● Preparation of post-construction notices to agencies as required by the project’s permits <br /> <br />DELIVERABLES: <br />● All documents shall be in PDF format unless otherwise specified <br />DocuSign Envelope ID: 30A47DAA-8ADC-4C81-A393-00F3D4407ED9
