Consulting Services Agreement between City of San Leandro and Exhibit A <br />GPA Consulting for Environmental Services for Lake Chabot Road Erosion Repairs Page 1 of 11 <br /> <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Project Description <br />Lake Chabot Road is a two-lane highway located along the southwestern slope of San Leandro Creek and <br />immediately downstream of Lake Chabot Dam. The roadway was constructed by cutting into the hillside upslope and <br />filling on top of the hillside downslope. During the January 2023 storms two separate erosion slides occurred, <br />namely site A and site B, making the roadway unsafe for vehicular traffic. <br /> <br />At site A, located 200' east of Chabot Terrace, approximately 80 linear feet of embankment below the road eroded <br />and slid downhill undermining the pavement edge of the westbound lane. Repairs are anticipated to include <br />construction of a drilled pier, retaining structure on the downslope, backfill and repaving of the westbound lane. <br /> <br />At site B, located 1600' east of Chabot Terrace, an approximate 100 linear feet section of upslope embankment slid, <br />depositing debris and rock slope protection (RSP) material onto the roadway below and exposing above dwellings to <br />structural vulnerability. Repairs are anticipated to include construction of a retaining wall at the toe of the slope, re- <br />grading of the upslope hillside with installation of salvaged and new RSP. <br /> <br />The City was awarded grant funding from the Federal Highway Administration to repair the roadway to support <br />vehicular traffic and return it to its pre-storm service condition. The grant will be administered locally through <br />Caltrans. <br /> <br />Scope of Services <br />This federally funded project will require environmental documentation pursuant to the National Environmental Policy <br />Act (NEPA) with Caltrans as the NEPA lead agency. The Consultant shall guide the project through the <br />environmental documentation process to achieve NEPA approval. All technical studies required for NEPA approval <br />shall be prepared in accordance with the Caltrans’ Standard Environmental Reference (SER) and Local Assistance <br />Procedure Manual (LAPM). <br /> <br />As an optional task, the Consultant will assist the City with meeting its Lead Agency requirements under <br />California Environmental Quality Act (CEQA). <br /> <br />The Consultant shall complete the work tasks and sub-tasks as listed below. <br /> <br />Task 1 – Project Description <br />• From review of relevant available data, a detailed project description shall be developed, including project <br />components, purpose and needs, environmental study limits (ESL) and timing of construction. The <br />project description will be appropriate for use in all technical studies and environmental documentation <br />and will include maps of the regional area and project area. <br />• Support will be provided to the City with grant applications and procedures. <br /> <br />Task 1 Deliverables: Detailed Project Description; ESL map <br /> <br />Task 2 – Preliminary Environmental Study <br />• Following Task 1, a draft Preliminary Environmental Study (PES) Form will be prepared. <br />• Attendance at a Caltrans field or virtual review of the project site shall be required. <br />Docusign Envelope ID: 78E72B1A-FA71-4376-9F02-D7D222FBE69F
