Exhibit A – Page 2 of 2 <br />The consultant shall evaluate the Request for services and in turn provide a Proposal. Acknowledgment <br />of the request should be completed no longer than 3 working days. Acknowledgement of the request <br />should include a time and day when proposal will be received after reviewing the request for services. <br /> <br />A Proposal should include a scope of services and a cost estimated which at the minimum should include: <br />1. City provided Project name, City Project Number and description <br />2. Deliverables required to complete assignment <br />3. Timeline <br />4. Expected Task list <br />5. Cost Estimate per Task <br />6. Not to exceed budget <br />7. Exemptions to work required for assignment <br />Acceptance will be provided via a Notice to Proceed, city staff will provide the consultant with any <br />additional documentation necessary to complete the work. These documents may include site plans, as- <br />builts, or other technical documents. The consultant will work with staff to ensure the necessary information <br />is attained to ensure final product is adequately prepared. <br />Lead consultant staff is required to attend meetings with staff, public or public hearings related to the <br />project when requested. All deliverables and work produced by the selected consultant(s), whether in hard <br />copy or electronic form, will become public record and the property of City within the limits of local enforcing <br />agencies or regulations. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />END OF EXHIBIT A
