ARTICLE 9. OTHER RECORD KEEPING AND REPORTING REQUIREMENTS <br />9.1 RECORD KEEPING <br />The Contractor shall maintain in its office records of the quantities (in Tons and cubic yards) of (i) Solid <br />Waste collected and disposed under the terms of this Agreement, (ii) Recyclable Materials, by type, <br />collected, purchased, processed, sold, donated or given for no compensation, and Residue disposed, and <br />(iii) Green Waste collected, received, purchased, processed, sold, donated or given for no compensation, <br />and Residue disposed. Said records shall be subject to the inspection provisions as provided in Article 8.2. <br />9.2 QUARTERLY REPORTING <br />Quarterly reports shall be submitted no later than the 25th of the month immediately following the end of the <br />quarter, with the first quarterly report submitted by July 25, 2000. Quarterly reports shall be submitted in <br />hard copy, and shall be provided electronically in a software and medium reasonably acceptable to the <br />City. <br />The Contractor shall submit the following quarterly reports to the City: <br />A. Solid Waste Data. Include separately for each Customer Class and for free City Facility service <br />the Tons collected and delivered to the Disposal Site. Indicate number of service accounts by service <br />classification level for residential and Commercial Customers. Indicate number of collection Containers <br />distributed by size and Customer type. <br />B. Recycling Data. Include gross Tons collected by material type for each Customer and for <br />facilities receiving free service. Include one week survey of participation rates during the quarter relative to <br />the total number of Customers in terms of weekly setout counts. Indicate, by material type (and grade <br />where appropriate), quarterly totals of Recyclable Materials processed and sold. Indicate any quantities, <br />by material type, donated or otherwise disbursed without compensation. Indicate number of collection <br />Containers distributed by size and Customer type. Reports shall include all data developed pursuant to <br />Article 4.13. C. Also report shall provide quarterly totals and location for Residue disposed. <br />C. Green Waste Data. Include average daily gross Tons collected. Include one week survey of <br />participation rates over the quarter relative to the total number of Customers in terms of weekly setout <br />counts. Indicate number of collection Containers distributed by size and Customer type, <br />D. Praises, Complaints, Missed Pickups. and Resolutions. Provide a copy of the log of praises, <br />Complaint Log, and resolutions of complaints, including a summary of the type and number of complaints. <br />Provide written record of all calls related to missed pickups and responses to such calls. <br />E. Problems and Actions Taken. Provide summary narrative of problems encountered with <br />collection and processing activities and actions taken. Indicate type and number of notification tags left at <br />Customer accounts.
