<br />. Employee Training on Hazardous Materials/ HazCom: <br /> <br />10 hours <br /> <br />I. Respiratory Protection Program: 32 hours <br />8, CCR 115144 <br />This written program will meet all regulatory requirements. This will provide respiratory <br />assessments including identification of all hazardous atmospheres, available engineering <br />and administrative controls, appropriate respiratory protection devices, medical <br />surveillance documentation*, and fit-testing. <br />. Employee Training & Fit-Testing on Respiratory Protection: 8 hours <br />(Fit-testing immediately following the training). <br /> <br />*Required medical surveillance form review is charged additionally at $25 each. On-site <br />doctor physical (Doc-in-the-box) for those who fail the medical form review is charged <br />additional $70 each (estimate 50 @ $25 & 15 @ $70). <br /> <br />J. HAZARDOUS MATERIALS BUSINESS PLAN (HMBP) 10 hours <br />Title 22, CCR, Division 4.5, Chapter 6.95 of the Health and Safety Code <br />This written program will be in compliance with State laws. Emergency responders are <br />provided with the necessary information to safely and appropriately respond to foreseeable <br />incidents such as: a fire, hazardous materials release or other emergency situations. <br />Hazardous materials and waste are properly managed (assignment of responsibility, <br />proper hazardous material containment, separation, etc.) The necessary spill equipment is <br />available and the employees have the necessary training to respond to a hazardous <br />material release. Du-AII Safety will identify on the new required Unidoc format the <br />designated locations of various hazardous materials and wastes that are above a <br />threshold amount. Recommend type and location(s) of emergency equipment. Emergency <br />contacts, update the site and facility maps which identify the locations of the hazardous <br />material and waste, emergency equipment locations, gas mains, water mains, storm <br />drains, etc. EPA identification numbers and the posting and placard numbering <br />requirements. <br />. Employee Training on Hazardous Waste / HMBP: <br />(Includes Emergency Response Guidebooks for each participant 24 hours <br /> <br />Part Two: Safety Maintenance and Consultation Needs: <br /> <br />A. Safety committee attendance: 1.5 hours per month 18 hours <br />B. Conduct semi-annual safety inspections of all employee areas at the City of San Leandro <br />C. Ergonomic workstation evaluations of employees who experience discomfort while on <br />the job (1 hour each). Included in Section I <br />D. New hire safety orientation class on the IIPP (1 per quarter) 1.5 each. 6 hours <br />E. Maintain office hours to assist & coordinate safety tasks including; management and/or <br />safety coordinator issues.
