Community input should inform the data points <br />to be measured. Outcomes should assess significant changes in <br />people's lives and well being, be reasonable within the systems' <br />influence, and be consistent across systems, Outcomes should also <br />correlate with legislated mandates for outcomes. Common data <br />: collection elements for systems should focus on maintaining housing <br />stability, improving quality oflife, and increasing self-sufficiency. <br /> <br />Strategy M-2-B: Establish baseline data. <br /> <br />Strategy M-2-C: Report and analyze outcomes regularly and use data to update action <br />plans. Ensure that each component of the system, as well as the <br />system as a whole, is accountable for its outcomes. <br /> <br />Strategy M-2-D: Funding should be tied to a program's performance. Outcomes will <br />help demonstrate whether each program and each system is meeting <br />the real needs of its consumers. <br /> <br />Strategy M-2-E: Develop a plan to communicate both specific positive outcomes and <br />overall progress towards goals to stakeholders and the community at <br />large, as well as to encourage understanding of-and participation <br />: in--efforts to prevent and end homelessness in Alameda County. <br />
