Gap Grant Program Application
Cycle #3 (06/07 - 07/08)

This proposed project is based on the need for same-day paratransit services
expressed in ACTIA's 2003 Paratransit Gap Service Needs Assessment. The City's
Paratransit Advisory Committee served as one of the key-Informant focus groups
that participated in this study. Additionally, focus groups were held at senior
centers, senior residential facilities and other community organizations serving older
adults or persons with disabilities througout Central Alameda County to obtain input
from current and potential paratransit consumers.

A multi-media marketing strategy will be used to promote the availability of this
new service. This will include, but Is not limited to: marketing materials consisting of
informational flyers, emall-notifications to organizations and professionals serving
paratransit consumers, telephone calls to targeted groups, mall stuffers, vehicle
signs, announcements on local City Cable channels, community billboards and
marquees, customized mailings to enrolled paratranslt program participants, "in-
vehicle" signs and handouts, and community presentations. A special effort will be
made to reach-out to non (and limited) English speaking communities that are
currently underserved.

G. Agency Coordination (10%): Please describe the extent to which you have
coordinated with other jurisdictions, public agencies, and community
transportation and/or social service providers to develop this proposal. Please
also describe how you will keep stakeholders involved and informed throughout
the project/program.

The Cities of Hayward and San Leandro are submitting separate, but coordinated,
applications to provide the same service through a common contracted service
provider. Additionally, the concept of an alternative to the taxi-model same-day
service has been discussed at PAPCO/ACTIA Technical Advisory Committee
meetings (involving paratransit consumers and City Paratranslt Program
representatives from throughout Alameda County) for over two years. Community
agencies served by City paratransit programs (inlcuding dialysis clinics, medical
offices, local hospitals, skilled nursing facilities and adult day care programs) have
been informed about, and support, the concept of offering same-day paratranslt
services in Central Alameda County. These key stakeholders will continue to be
involved in, and informed about, the project via participation on the City's
conumer-based Paratransit Program Advisory Committee and through the
marketing strategies described above.

H. Sustainability (8%): Please explain what other revenue sources you are currently
pursuing or planning to pursue to ensure sustainability of this project/program
beyond the funding term of a gap grant, including long-term efforts.

G:\Oliver\Paratransit\FY 2006.07\Gap Application.
