Out-of--Town Medical Trips are provided for medical purposes only outside of the <br />CITY limits and within Alameda County to eligible riders. Curb-to-Curb Service <br />eligibility requirements apply. A trip may be scheduled up to 5 days in advance. <br />Rides are reserved on a first call, first serve basis. Same-day reservations are to be <br />accepted as available. <br />1.0.4. Interim Service and Urgent Trips <br />The CITY provides interim service or urgent trips on a case-by-case basis to <br />eligible individuals who are not yet enrolled with East Bay Paratransit or the CITY <br />Program. Interim service may include subscription rides. CITY authorization is <br />required for interim service. <br />1.0.5. Group Trips <br />Recreational Group Trips for seniors and people with disabilities are offered <br />through the City's Senior Services Program. Approximately: <br />(12) Out and About Community Based Trips, 45 passenger vehicle <br />(8) Travel Troupe Trips, 45-passenger vehicle with restroom facilities <br />(I6) Senior Day Trips, 25-passenger vehicle <br />Vehicle must be wheelchair accessible. For larger vehicles that are not lift <br />equipped, the driver must have the ability to transfer a wheelchair passenger onto <br />the vehicle and into a seat. Trips offered Monday through Sunday within the San <br />Francisco Bay Area. CITY will make advance reservation for the trip through the <br />CONSULTANT. <br />ELIGIBILITY REQUIREMENTS <br />Who can use the Flex Shuttle Bus Service? <br />A resident of the City of San Leandro who is: <br />60 years old or older, or 18 years or older and East Bay Paratransit Certified <br />Who can use the FIeX Shuttle Curb-to-Curb and Out-of Town Medical Services? <br />A resident of the City of San Leandro who is: <br />75 years old or older, or l 8 years or older and East Bay Paratransit Certified. <br />The Flex Shuttle is intended for seniors and persons with disabilities. The C1TY will determine <br />eligibility criteria, assess applications and provide the CONSULTANT with updated registration <br />list for trip booking. All registrants will be issued a registration card for presentation to shuttle <br />bus drivers upon boarding. The CONSULTANT will be responsible for the verification of all <br />trip requests to ensure eligibility. <br />Eligible passengers will be able to travel with anon-registrant companion to assist with parcels <br />and act as a Personal Care Attendants (PCA) if the registrant feels one is required.
