City of San Leandro <br />Community Benefit RFP <br />Proposal Instructions <br />Page IV-1 <br />Community Benefit Grant Funds <br />Request for Proposals <br />2008-09 <br />SECTION IV <br />PROPOSAL INSTRUCTIONS <br />Be sure to attach the completed Proposal Checklist as the first page of your original proposal. <br />The next page of your proposal should be Form 1 -Cover Sheet. Be sure to complete the <br />summary. The summary should include: <br />A. A description of the project; <br />B. The total number of anticipated residents expected to attend/participate <br />The Executive Director must sign the eligibility statement at the bottom of the page. To be eligible <br />the proportion of San Leandro residents served must be equal to or greater than the proportion of <br />San Leandro funds that will be used to support this project. Also, each applicant should be a non- <br />profit organization, which has received approval of State tax exemption, or, at a minimum have an <br />application pending before the TRS in order to qualify as atax-exempt nonprofit organization. Both <br />public and private nonprofit organizations may apply. <br />In addition, every applicant should show evidence of having a board of directors and/or advisory <br />body which provides advice regarding the type, quality, and appropriateness of program(s) that are <br />to be funded by the City of San Leandro. <br />
