City of San Leandro <br />Community Benefit RFP <br />Proposal Instructions <br />Page N-2 <br />Proposal Narrative (1Vo Form) <br />Prepare a proposal narrative of a maximum of two (2)-typed pages. If your proposed project is <br />small or simple, one (1) page will suffice. Attach this narrative after Section I of the proposal. <br />Divide the narrative into three parts: <br />1. Introduction (1/2 page maximum) <br />2. Project Description (1 page maximum) <br />3. Evaluation (1/2 page maximum) <br />Please do not go over two (2) pages. <br />Narrative Part 1: Introduction (1/2 page) <br />A. Briefly provide the following background information on your organization: <br />* Mission/goal of overall organization. <br />* Number of years organization has been in existence. <br />* Significant past accomplishments. <br />Narrative Part 2: Project Description (1 page) <br />Briefly describe your project by documenting specific needs in the community which your <br />project will address. Explain how your program will promote San Leandro as ahigh-spirited, <br />family friendly and culturally diverse community. <br />Narrative Part 3: Evaluation (1/2 page) <br />Tell how you will accurately keep an account of the number of residents who attend your <br />program. Explain how you will survey those attending to determine the overall response to <br />your program. <br />
