City of San Leandro <br />Community Benefit RFP <br />Froposal Instructions <br />Page IV-3 <br />BUDGET: INCOME SOURCES (Form 2) <br />Section A -Line Item Chart of Income Sources <br />The information you provide in this section depends on your particular situation. In some <br />cases, San Leandro funds are being requested to help support a specific project or program <br />within an organization. If this describes your situation, you are asked to provide information <br />about the income sources that are associated with this project or program only. <br />If on the other hand San Leandro funds are being requested to support all of the programs that <br />are provided by the organization, then you are asked to provide information about all of the <br />income sources that are available to the entire organization. <br />In either case, list all sources from which you reasonably expect to receive funds. Indicate, in <br />the appropriate column, the name of the source and the amount expected from each source. <br />Section B -Leverage Ratio <br />Calculate the ratio of other funds to funds requested from San Leandro. To do so, add the <br />amounts requested from other sources (enter on line A) and divide this amount by the total <br />amount of funds requested from San Leandro for this project (line B). Enter the resulting <br />figure on line C (Leverage Ratio). <br />NOTE: Only one (1) original (no copies) of each document needs to be submitted. <br />
