SECTION II <br />GENERAL INFORMATION <br />A. Introduction: <br />The City of San Leandro's Recreation and Human Services Department is issuing this Request <br />for Proposal (RFP) to organizations interested in applying for the City of San Leandro General <br />and Community Development Block Grant (CDBG) funds to support social service programs <br />serving San Leandro residents during the period July 1, 2008 through June 30, 2009 and July 1, <br />2009 through June 30, 2010. The City has prepared this application package to inform interested <br />parties of the minimum requirements, outline the assessment criteria, and assist applicants in <br />presenting their proposals. <br />B. Availability of Funds: <br />Contingent on the availability of funds, the City of San Leandro will make available an <br />estimated $400,000, comprised of General Funds and CDBG funds for FY 2008-09 and <br />400 000 for FY 2009-10 to support social service programs. Currently, the City supports <br />twenty-one (21) social service programs with allocations ranging from $5,000 to $40,000 per <br />year. Funds will be granted for the two-year grant cycle, contingent on available funding. <br />Second-year funding will not require a new grant application. <br />Community Assistance funding is limited to a single organization at a maximum grant of <br />$40,000. An exception to this may be granted on the basis of availability of additional funds <br />and/or a program that meets an immediate and critical human need in which funding above <br />$40,000 may be necessary to implement the program. <br />The services provided under grant contract may be terminated at any time without cause. In that <br />event, the City would compensate the grantee for work satisfactorily completed as of the date of <br />the written notice. <br />C. Conditional Release of Second Year Funds: <br />The release of second-year funding will be contingent upon compliance with contract <br />requirements, timely submittal of required reports and the availability of funds. <br />D. Grant Monitoring: Semi-Annual Reports, Site Visits and Directors Meetings <br />Agencies receiving grant funds will be required to submit semi-annual reports in the format <br />provided by the City. Please note the reporting categories listed in the RFP. Agencies <br />receiving funding will be required to report statistical information in the categories listed. <br />Members of the Human Services Commission and staff will conduct at least one site visit per <br />year. Agency representatives are encouraged to attend the quarterly Directors Meetings. <br />E. CARD Emergency Preparedness Training <br />Agencies receiving grant funds will be required to participate in the City of San Leandro and <br />Collaborating Agencies Responding to Disasters (CARD) emergency preparedness training.
