EXHIBIT A <br />SCOPE OF WORK <br />Term of Contract: July 1, 2009- June 30, 2010 <br />Agency: Davis Street Family Resource Center <br />Address: 3081 Teagarden Street <br />San Leandro, CA 94577 <br />Contact Person: Rose Padilla Johnson, Executive Director <br />Phone: 510-347-4620 <br />Project Name: Comprehensive Family Support Services <br />PROJECT GOALS AND OBJECTIVES <br />The Family Support Services Program will provide food, clothing, information regarding PG8~E energy <br />assistance, job attainment and retention services, and linkages to long-term assistance such as childcare and <br />health services for low-income, families, individuals and seniors of San Leandro, thereby. reducing the risk of <br />compromising their self-sufficiency. <br />MAJOR ACTIVITIES <br />Families will receive a three day supply worth of groceries up to two times a month (canned goods, bread, <br />produce, dairy and meat products), and other services as needed, including: clothing/household items, PG&E <br />utility assistance enrollments, employment and housing assistance, information and referral to services not <br />provided (such as rental assistance, shelter, affordable health care plans, etc), employment counseling, <br />internal linkages to mental health counseling and childcare, free acute medical care and primary health care. <br />Referrals are made to other agencies for specialized services such as housing and additional food resources. <br />PERFORMANCE MEASURES <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT use evaluation tools that consist of a client <br />intake interview, follow-up with subsequent visit(s) and re-certification, and documentation of frequency of <br />visits. To track this grant's outcomes, Subrecipient will monitor client's housing and employment status. The <br />performance measures are: <br />1. Six hundred (600) families will be served each month, totaling 7,200 families served within twelve (12) <br />months. <br />2. Sixteen thousand two hundred meals (16,200) meals will be provided per month, totaling 194,400 <br />within (12) months. This calculation is based on a 3 day supply (9 meals) per individual, estimating that <br />the average household is comprised of 3 individuals. <br />-14- <br />CDBG Agreement with Davis Street Family Resource Center FY09-10 <br />
