EXHIBIT A <br />SCOPE OF SERVICES <br />Term of Contract: July 1, 2005 -June 30, 2006 <br />Agency: Building Futures with Women and Children <br />Address: 1395 Bancroft Avenue <br />San Leandro, CA 94577 <br />Contact Person: Carole Hay, Contracts Manager <br />Phone: 510-357-0205 (X-129) <br />Project Name: San Leandro Shelter <br />PROJECT GOALS AND OBJECTIVES <br />The San Leandro Shelter will undergo major rehabilitation and improvements to its facility. This renovation <br />project will not only address deferred maintenance issues such as replacing the roof, windows and furnace, it <br />will improve the function of the building to support program operations such as food preparation, children's <br />services and waste management. It will also make the shelter a more welcoming positive environment for the <br />traumatized women and children who come to stay there. This CDBG capital improvement grant will fund the <br />cost of replacing the roof, painting the building exterior, rebuilding the trash and recycle enclosures, and <br />installing a trellis at the entry to the court yard. <br />The San Leandro Shelter serves 280-350 homeless women and children per year. These improvements will <br />make the program an even more positive and supportive place in which these families can begin to rebuild <br />their lives. <br />MAJOR ACTIVITIES and PERFORMANCE MEASURES <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT: <br />The San Leandro Shelter will have a new roof, new building exterior paint, rebuilt trash and recycle enclosures, <br />and anew trellis at the entry to the court yard funded by CDBG funds, along with other major renovations <br />funded by other sources. <br />REPORTS AND MISCELLANEOUS <br />1. For CDBG funds, Agency must submit quarterly and annual reports using the City's reporting form <br />within 15 days of the end of the quarter on the number of persons served. Quarterly reports shall include a <br />narrative section and shall be submitted even if there are no specific numbers to report.
