The Contractor will be expected to identify the services for which the household is eligible and in need <br />of in the housing and income stabilization plan, and to document and report on whether services were <br />successfully secured. <br />7. Data Collection in HMIS and participation in Outcome Evaluation <br />As part of this contract, information about client households and the HPRP-funded services that they <br />receive will be entered into the county-wide In-HOUSE homeless management information system <br />(HMIS.) Contractor must comply with all data collection and entry requirements. <br />8. Oversight and Reporting <br />The Contractor must include appropriate supervision to be able to oversee the case management and <br />housing placement activities or functions; review and approve applications for HPRP financial <br />assistance and process payments; and prepare monthly reports for the City, State and Everyone Home. <br />9. Participation in Policy and Oversight Committee and in County-wide "Learning <br />Community" <br />The intent of this coordinated effort is to improve the way in which housing crises are identified and <br />addressed in our community, including increasing our system's ability to prevent homelessness and to <br />more quickly re-house those who become homeless. Because this effort is new and is being put in to <br />place in a very short time frame, it is anticipated that modifications will be made during the course of the <br />contract period. The agency will participate in a countywide coordinating body and learning <br />community. This group will meet on a regular basis to share information and provide feedback about <br />how the program is working in each region, provide feedback on the common program elements such as <br />the referral process, assessment tool and data collection efforts, and facilitate communication and <br />coordination between providers and jurisdictions. <br />Population to be Served: Households eligible for HPRP-funded financial assistance and/or services <br />maybe individuals (youth, adults) and/or families. They must meet all of the following criteria: <br />• Income. HUD requires that household income be no greater than 50% of Area Median Income. <br />Local priority has been established for those below 30% of Area Media Income. <br />Homeless or At Risk of Homelessness. For purposes of HPRP, HUD's definition of homeless is <br />the same as in other McKinney-Vento homelessness programs. HUD is allowing localities to <br />define at-risk of homelessness and is encouraging targeting based on known risk factors. <br />Jurisdictions in Alameda County receiving funding under the HPRP have agreed to utilize a <br />common definition for at-risk of homelessness which will be assessed through a common <br />assessment tool. The Contractor agrees to utilize the County-wide definitions to assess eligibility. <br />• No ,O~tions or Resources: (1) No appropriate subsequent housing options have been identified, <br />and (2) The household lacks the financial resources and support networks needed to obtain <br />immediate housing or remain in its existing housing. <br />Ex.
