Homelessness Prevenrion and Re-housing 3 October 5, 2009 <br />Program Plan MOU <br />linking program participants to community resources and mainstream benefits, and helping <br />them develop a plan for preventing future housing instability. <br />Financial Assistance <br />Financial assistance is limited to the following activities: rental assistance (short- and <br />medium-term rental subsidies); rent arrears; security deposits; utility deposits; utility <br />payments; moving cost assistance; and, motel and hotel vouchers. <br />The Mid-County Housing Resource Center <br />The Mid-County HRC is a joint collaboration among the cities of Alameda, Hayward and San <br />Leandro and Alameda County HCD, along with key regional and local service providers. <br />Services will be provided to residents and homeless people of Alameda, Hayward, San <br />Leandro, and adjacent unincorporated county areas. The service partners include Building <br />Futures with Women and Children (BFWC) as the lead agency, Abode Services, Davis Street <br />Family Resource Center (DSFRC), and ECHO Housing. Vendors include Eden Information <br />and Referral, which will provide 2-1-1 services and a roving housing specialist for Hayward. <br />Mid-County HRC's housing and rapid re-housing services are partially funded for three years <br />by the cities of Alameda and Hayward and Alameda County, all entitlement grantees. The <br />Mid-County HRC will have approximately $1.7 million from Alameda, Hayward, and <br />Alameda County, which are all entitlement grantees, to spend for HPRP activities. BFWC's <br />HPRP grant funding of $1.5 million must be spent on eligible San Leandro residents. <br />The City coordinated mid-county jurisdictional meetings to elicit input for program design <br />and budgets. The bulk of the funding will be spent on financial assistance for mid-county <br />residents, and the remainder will be allocated for housing stabilization services to help <br />residents maintain or attain housing. The HRC will also be able to refer clients to or access <br />funds from other homelessness prevention and re-housing sources, including SSA/TANF, <br />Behavioral Health Care, FEMA and Season of Sharing. The Mid-County HRC goal is to serve <br />over 600 HPRP clients over three years. The City of San Leandro's three-year goal under the <br />Mid-County HRC is to serve approximately 200-300 families and 150-194 unduplicated <br />individuals who reside in the City. <br />The Mid-County HRC will be located at Davis Street Family Resource Center (DSFRC). <br />DSFRC was chosen because of the variety of programs offered in their multi-service agency, <br />their long-standing connection to communities in need, their capacity to provide HPRP- <br />related services, and experience in serving low-income bilingual (English/Spanish) <br />populations. The HRC will be open five days a week. The cities of Alameda and Hayward <br />will also offer housing resource services locally to ensure such services are easily accessible <br />to their homeless and low-income residents. <br />Linkages to Other Resources <br />HRCs are not designed to provide the entire breadth of services that households may require <br />in achieving housing stability. Although the Mid-County HRC has extensive in-house <br />services, it also has systems in place to refer clients to other services as needed. A case <br />manager will broker such services and assist the client to access them as needed. These <br />
