EXHIBIT "D" <br />MITIGATION MONITORING AND REPORTING PROGRAM <br />FOR THE KAISER SAN LEANDRO MEDICAL CENTER / MIXED-USE RETAIL DEVELOPMENT PROJECT <br />Monitoring Responsibility Implementation and <br />Environmental Impact Mitigation Measures Monitoring Timeline <br />shall provide a transportation coordinator who will be responsible for organizing <br />and disseminating information on alternative transportation modes to tenants and <br />shopping center guests. The employee shall have had previous experience with <br />TDM and shall be trained for the role. <br />A website shall be one of the programs organized by the Transportation <br />Coordinator, containing information on parking, transportation, and TDM - <br />related information. <br />b. A transit information fair shall be held quarterly (four times per year), and then <br />can be reduced to occur annually (once per year) once the identified single - <br />occupant vehicle reduction target has been met. <br />• Shuttle Service — The Mixed -Use Retail Development project sponsor shall <br />coordinate with the Kaiser Medical Center to supplement the Kaiser Medical <br />Center's shuttle service between the San Leandro BART Station and the project <br />site. Alternatively, if another public or private shuttle program exists that provides <br />shuttle service between the San Leandro BART Station and the project site, then <br />the Mixed -Use Retail Development project sponsor may participate in that <br />program and shall pay their fair share of the costs of that program. <br />• Transit Subsidies — The project sponsor for the Mixed -Use Retail Development <br />shall promote the use of transit subsidies by shopping center tenants/employers <br />for all feasible transit modes. The Transportation Coordinator for the Mixed -Use <br />Retail Development shall provide information on transit subsidy programs to the <br />shopping center tenants. <br />• Guaranteed Rides Home — The project sponsor for the Mixed- Use Retail <br />Development shall participate in the Alameda County Guaranteed Ride Home <br />Program established by the ACCMA, which guarantees a ride home for all <br />carpool, vanpool, and transit users. <br />• The project sponsor for the Mixed -Use Retail Development shall provide bicycle <br />storage facilities on the project site. <br />• Employee TDM survey—An initial survey of the Mixed -Use Retail Development <br />employees shall be conducted to identify potential areas of focus for the <br />marketing of TDM to the Mixed- Use Retail Development employees. The survey <br />shall identify the various means of transportation to work used by employees, the <br />quantities of employees using each, and the improvements that can be made to <br />the TDM program. Should it be determined that the TDM program objectives are <br />not being achieved, then the applicant and the City shall meet to jointly determine <br />the feasibility of additional or modified measures designed to achieve this <br />objective. The City Community Development Director and Engineering and <br />Transportation Director shall have the final authority and discretion (subject to <br />appeal to the Planning Commission) to determine additional or modified measures <br />that may be needed to achieve the TDM reduction target.
