The items of work which the bidder made available to DBE firms, including, where appropriate, any breaking <br /> down of the contract work items (including those items normally performed by the bidder with its own forces) <br /> into economically feasible units to facilitate DBE participation. It is the bidder's responsibility to demonstrate <br /> that sufficient work to meet the DBE goal was made available to DBE firms. <br /> D. The names. addresses and phone numbers of rejected DBE firms, the firms selected for that work, and the <br /> reasons for the bidder's choice. <br /> E. Efforts made to assist interested DBEs in obtaining bonding, lines of credit or insurance, and any technical <br /> assistance or information related to the plans, specifications and requirements for the work which was provided <br /> to DBEs. <br /> F. Efforts made to assist interested DBEs in obtaining necessary equipment. supplies, materials, or related <br /> assistance or services, excluding supplies and equipment the DBE subcontractor purchases or leases from the <br /> prime contractor or its affiliate. <br /> G. The names of agencies contacted to provide assistance in contacting, recruiting and using DBE firms. <br /> H. Any additional data to support a demonstration of good faith efforts. <br /> • <br /> SECTION 3. AWARD AND EXECUTION OF CONTRACT <br /> The bidder's attention is directed to the provisions in Section 3, "Award and Execution of Contract," of the <br /> Standard Specifications and these special provisions for the requirements and conditions concerning award and <br /> execution of contract. <br /> (Use the following paragraph if DBE goals are specified) <br /> The award of the contract. if it be awarded. will be to the lowest responsible bidder whose proposal complies <br /> with all the requirements prescribed and who has met the goal for DBE participation or has demonstrated, to <br /> the satisfaction of the City of San Leandro, adequate good faith efforts to do so.
