All were certified to use Alameda County's <br />HMIS-based software InHouse. Two (2) staff members also received the Advanced <br />Reporting Tool (ART) certification which allows them to cokkreports. The DSFRC <br />HMIS program is being implemented successfully altho F slowly due to the lengthy <br />intakes required and the amount of data that needs to be d and entered. DSFRC is <br />now in possession of fifteen (15) scanners, one for e.. proGeneral Intake, Food, <br />Clothing, Rota Care Adult Clinic, Rota Care P diic Clm primary care Clinic, <br />Podiatry clinic, Dental, Physical Therapy,t55��eling, APP; ,�rson, Garfield, <br />Roosevelt, and 1190. Build ct ter jr- <br />ome., <br />ctional cooperation and further coordination <br />and imement of tContinuum of Care System. <br />Activities. <br />♦ EveryOne <br />EveryOne Home is O'k u nity-based organization formed to coordinate the implementation of <br />the EveryOne Home P, (formerly known as the Alameda County Homeless and Special Needs <br />Housing Plan). The I✓veryOne Home Plan is a comprehensive blueprint to end homelessness, <br />including chronic homelessness, by the year 2020, and address the housing needs of extremely <br />low income persons living with serious mental illness and or HIV/AIDS. The Plan employs five <br />(5) core strategies: <br />1. Prevent homelessness and other housing crises; <br />2. Increase housing opportunities for the Plan's target populations by 15,000 units; <br />3. Deliver flexible services to support stability and independence; <br />4. Measure success and report outcomes; <br />5.
