EXHIBIT A <br /> SCOPE OF WORK <br /> • <br /> Term of Contract: July 1, 2012 — June 30, 2013 <br /> Agency: Girls Incorporated of Alameda County <br /> Address: 13666 East 14 Street <br /> San Leandro, CA 94706 <br /> Contact Person: Rebecca Cannon, Chief Mental Health Officer <br /> Phone: 510- 357 -5515 <br /> Project Name: Pathways Counseling Center's Family Strengthening Program <br /> PROJECT GOALS AND OBJECTIVES <br /> The Pathways Counseling Center's Family Strengthening Program will provide individual and family therapy <br /> services and Parent/Family Workshops that enable individuals and families to address psychological and <br /> communication problems within their families thus increasing the likelihood that these problems do not escalate <br /> into parental abuse or adolescent acting -out behavior. The Family Strengthening Program will also include <br /> sharing community resources to help families to access health and other family - related services. The program <br /> will also provide family crisis intervention services, in which children and families can engage in order to <br /> develop communication skills and psychological capacity to overcome the current "hot" crisis of their families <br /> and reunite as a family, and respite care intervention for pre - delinquent and delinquent youth. Services will be <br /> delivered in Spanish and English. <br /> MAJOR ACTIVITIES <br /> Pathways services are: Individual and family therapy services as well as consultation services with other <br /> professionals involved in the cases (e.g., teachers, school administrators, pediatricians, psychiatrists, etc.) <br /> Mental Health Clinicians will conduct clinical assessments (measured on a 10 -point scale conducted at: intake, <br /> subsequent 90 -day intervals, and termination). Participating family members will complete communication <br /> skills assessments as well as survey evaluations regarding quality of treatment services. <br /> Many additional services are delivered for San Leandro residents beyond those services funded by the City of <br /> San Leandro. Those additional Family Strengthening Program services are listed below. Girls Inc. will provide <br /> a summary of these services in the narrative section of the quarterly reports. <br /> 1. Family crisis intervention (in conjunction with the Alameda Co. Crisis Receiving Home, Malabar House). <br /> 2. Respite care intervention for pre - delinquent and delinquent youth. <br /> 3. Effective Parenting Workshops <br /> Mental Health Clinicians conduct family functioning matrices as well as child behavioral assessments. <br /> Program participants complete survey evaluations regarding quality of crisis intervention and/or respite <br /> intervention services. <br /> -13- <br /> CDBG Agreement with Girls Inc.
