PERFORMANCE MEASURES <br /> SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT <br /> 1. Provide individual and family therapy services and /or consultations services for fourteen (14) San <br /> Leandro children and family members served within twelve (12) months. <br /> 2. Assist 11 of the 14 participating children and family members to address psychological and <br /> communication problems within their family in order to alleviate parental abuse and /or child acting -out <br /> behavior. <br /> TIMELINES <br /> By October 1, 2012, Girls Incorporated of Alameda County will have: <br /> • Served three (3) participating children and family members with individual therapy, family therapy, <br /> and /or consultation services. <br /> • Demonstrated that two (2) of the three (3) participating children and family members will be able to <br /> address psychological and communication problems within their family in order to alleviate parental <br /> abuse and /or child acting -out behavior. <br /> By January 1, 2013, Girls Incorporated of Alameda County will have: <br /> • Served six (6) participating children and family members with, individual therapy, family therapy, and /or <br /> consultation services. <br /> • Demonstrated that four (4) of the six (6) participating children and family members will be able to <br /> address psychological and communication problems within their family in order to alleviate parental <br /> abuse and /or child acting -out behavior. <br /> By April 1, 2013, Girls Incorporated of Alameda County will have: <br /> • Served ten (10) participating children and family members with individual therapy, family therapy, <br /> and /or consultation services. <br /> • Demonstrated that seven (7) of the ten (10) participating children and family members will be able to <br /> address psychological and communication problems within their family in order to alleviate parental <br /> abuse and /or child acting -out behavior. <br /> By July 1, 2013, Girls Incorporated of Alameda County will have: <br /> • Served fourteen (14) participating children and family members with individual therapy, family therapy, <br /> and/or consultation services. <br /> • Demonstrated that eleven (11) of the fourteen (14) participating children and family members will be <br /> able to address psychological and communication problems within their family in order to alleviate <br /> parental abuse and /or child acting -out behavior. <br /> - 14 - <br /> CDBG Agreement with Girls Inc.
