EXHIBIT A <br /> SCOPE OF WORK <br /> Term of Contract: July 1, 2012 — June 30, 2013 <br /> Agency: Safe Alternatives to Violent Environments (SAVE) <br /> Address: 1900 Mowry Avenue, Suite 204, Fremont, CA 94538 <br /> Contact Person: Rodney Clark, Executive Director <br /> Phone: 510 - 574 -2250 <br /> Project Name: SAVE — Teen Dating Violence Prevention Program <br /> PROJECT GOALS AND OBJECTIVES <br /> SAVE's Teen Dating Violence Prevention Program works to prevent and reduce domestic violence by <br /> providing prevention services in the schools. SAVE promotes alternatives to domestic violence through support <br /> services, advocacy, and education. SAVE also helps domestic violence victims and their families end the <br /> cycle of violence by providing crisis intervention services that include crisis counseling, domestic violence <br /> education, case management, and restraining order assistance. <br /> MAJOR ACTIVITIES <br /> SAVE will provide Teen Dating Violence Presentations to middle and high schools throughout the Alameda <br /> County. The presentations provide middle and high school students with information about domestic violence, <br /> including the dynamics of healthy vs. unhealthy intimate relationships, thus empowering them with knowledge <br /> that promote their well -being and actively striving to end the cycle of violence. <br /> PERFORMANCE MEASURES <br /> SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT <br /> 1. Have 250 students from middle and high schools in San Leandro and surrounding communities <br /> participate in a Teen Dating Violence Prevention Presentation. <br /> 2. Increase the knowledge of domestic violence and the resources available to domestic violence victims <br /> to seventy -five percent (75 %) (188 of 250) of the students that participate in the Teen Dating Violence <br /> Prevention Presentations. <br /> 3. Provide crisis intervention services, domestic violence education, and /or referrals to 125 Alameda <br /> County residents who are domestic violence victims and have contact with law enforcement. <br /> 4. Increase the level of self- sufficiency of seventy -five percent (75 %) (94 of 125) of the Alameda County <br /> residents, who are victims and have made domestic violence reports, by providing them crisis <br /> intervention services and domestic violence education which teach them new safety skills. <br /> - 13 - <br /> CDBG Agreement with SAVE FY12 -13 <br />
