HAZARDOUS MATERIALS PROGRAM <br /> An employee assigned to the Alameda County Fire Department Hazardous Materials <br /> Team shall be compensated an additional 5% of the base salary for his/her <br /> classification indicated in Appendix A, at such time the Fire Department implements <br /> a Hazardous Materials Program or participates with other agencies in an existing <br /> hazardous materials program. <br /> SECTION 13. HEALTH AND DENTAL PLANS <br /> A. HEALTH PLAN COVERAGE <br /> 1. For coverage from January 1, 1994 through January 31, 1995, the <br /> County/District shall contribute towards the monthly provider's charge for a <br /> comprehensive group health plan by a health maintenance organization or <br /> toward an indemnity health plan for eligible full-time employees, as well as <br /> their spouses and eligible dependents up to $173.04 per month for an employee <br /> with no dependents; up to $346.08 per month for an employee with one <br /> dependent; up to $460.28 per month for an employee with two or more <br /> dependents. This contribution shall apply to the health plan options listed <br /> below. <br /> • <br /> a. Indemnity Options: <br /> • <br /> (1) Blue Cross Prudent Buyer (identified as H -2 in the employee <br /> handbook) <br /> (2) Spousal Coverage Plan (identified as H -3 in the employee <br /> handbook) <br /> b. Kaiser (identified as H -4 in the employee handbook) <br /> c. California Care (identified as H -5 in the employee handbook). <br /> d. Health Net - Plan T (identified as H -6 in the employee handbook). <br /> 2.
