SECTION 12. DrikARTMENT CONTRIBUTION TO ARD RETIREE HEALTH PLAN <br /> COVERAGE (Cow) <br /> P.E.R.S. Plans: <br /> For coverage effective January 1, 1995, Department employees who have already retired or who <br /> thereafter retire, under the Public Employees' Retirement System and PERS' Health Benefits <br /> Program, will be entitled to a Department contribution for retirees up to a maximum of $171.81 <br /> per month for the 1994 -95 fiscal year. In no event shall the Fire Department's contribution for <br /> retiree coverage exceed the premium of the option selected. The Department's contribution for <br /> each retiree/annuitant shall thereafter be increased annually by five percent (5%) of the monthly <br /> contribution for employees until such time as the contributions for active employees an.d <br /> retiree/annuitants with the same family status (single, two -party or family) are equal. <br /> • <br /> SECTION 13. VISION REIMBURSEMENT <br /> Employees are eligible for vision care reimvursement subject to the following criteria: an <br /> employee is eligible for such reimbursement after six months'continuous employment, working <br /> at least 50% or more time. Employees are reimbursed for the cost of either lenses and frames or <br /> contact lenses specifically prescribed for the employee only, up to a maximum of $100 in the <br /> twenty -four month period ending December 31, 1995. <br /> SECTION 14. LONG TERM DISABILITY INSURANCE PLAN <br /> The Department shall pay an additional $12.00 in salary as specified in Appendix A of this <br /> Memorandum of Understanding to offset the cost of the firefighters participation in a long -term <br /> disability insurance plan through the California Union of Professional Firefighters. <br /> SECTION 15. FIRE TRAINING INCENTIVE AWARD PROGRAM <br /> The parities agree to continue in full force and effect the Fire Training Incentive Award Program <br /> as outlined in detail in Appendix B of this Memorandum. <br /> SECTION 16. SAFETY EQUIPMENT <br /> The Department shall provide the following safety equipment: <br /> turnout boots <br /> turnout coat • <br /> turnout pants <br /> helmet with visor • <br /> OSHA- approved leather boots <br /> gloves <br /> NFPA 1500 approved uniform pant, when replacements are necessary <br /> L1FF Local 142 8-Management Unit R34 <br /> January 1, 1994- December 31, 1995 <br /> 16 <br /> 2 8 4 Local 1428nvn11-2 <br /> RUJm•N1 <br />
