The hospital’s or clinic’s governing line 2 board shall document that it has reviewed the clinic’s line 3 organizational mission statement and considered amendments to line 4 it that would better align that organizational mission statement line 5 with the community benefits statement. line 6 (2) A description of the process for approval of the community line 7 benefits statement by the hospital’s or clinic’s governing board, line 8 including a declaration that the board and administrators of the line 9 hospital or clinic shall be responsible for oversight and line 10 implementation of the community benefits plan. The board may line 11 establish a community benefits implementation committee that line 12 shall include members of the board, senior administrators, and line 13 community stakeholders. line 14 (3) A community health needs assessment pursuant to Section line 15 127476 that evaluates the health needs and resources of the line 16 community it serves. line 17 (c) By April 1, 2015, a private nonprofit hospital or nonprofit line 18 multispecialty clinic shall develop, in collaboration with the line 19 community, a community benefits plan pursuant to Section 127477 line 20 designed to achieve all of the following outcomes: line 21 (1) Access to health care for members of underserved and line 22 vulnerable populations. line 23 (2) The addressing of essential health care needs of the line 24 community, with particular attention to the needs of members of line 25 underserved and vulnerable populations. line 26 (3) The creation of measurable improvements in the health of line 27 the community, with particular attention to the needs of members line 28 of underserved and vulnerable populations. line 29 127476. (a) Prior to adopting a community benefits plan, a line 30 private nonprofit hospital or nonprofit multispecialty clinic shall line 31 complete a community needs assessment that evaluates the health line 32 needs and resources of the community served by the hospital or line 33 clinic that is designed to achieve the outcomes specified in line 34 subdivision (c) of Section 127475. line 35 (b) In conducting its community health needs assessment, a line 36 private nonprofit hospital or nonprofit multispecialty clinic shall line 37 solicit comments from and meet with local government officials, line 38 including representatives of local public health departments.
