• • <br /> (i) The Fire Employee shall have reached the age of 47 as of the date of the <br /> effective date; and <br /> (ii) The Fire Employee shall have completed 17 years of service with the San <br /> Leandro Fire Department as of the effective date. <br /> ACFD shall pay the premium for such insurance plan up to the dollar amount it would <br /> pay if the employee had selected option (a) and City shall pay the premium in excess of such <br /> amount. Fire employee selecting option 3(a) will not be eligible to enroll in the City's <br /> retirement plan at a later date. <br /> 4. Termination of Retirement Transition Provision - The retirement option described in <br /> Section 3(J)(3) shall remain in effect until: <br /> a. The ACFD employer contribution rate for retiree medical insurance equals the two <br /> party rate of the Kaiser North premium in the PERS medical plan or: <br /> b. Three years from the effective date, whichever occurs first. <br /> Thereafter, City shall have no liability for medical insurance for Fire Employees retiring from <br /> ACFD after such date. <br /> 5. Transition of Retirement Dental Plan-Retiring Fire Employees and eligible dependents <br /> may re-enroll in City's dental plan, subject to the approval of the dental insurance carrier, at <br /> the City's contribution rate in effect for retirees of other City bargaining units and may remain <br /> enrolled until age sixty-five, subject to the following requirements: <br /> a. The Fire Employee shall have reached the age of 47 as of the effective date; and <br /> b. The Fire Employee shall have completed 17 years of service with the San Leandro Fire <br /> Department as of the effective date. <br /> 6. Termination of Retirement Transition Provision - The retirement option described in <br /> Section 5 (a) (b) shall remain in effect until: <br /> a. The ACFD makes available a retirement dental plan; or: <br /> b. Three years from the effective date; whichever occurs first. <br /> Thereafter, City shall have no liability for dental insurance for Fire Employees retiring from <br /> the ACFD after such date. <br /> 7 <br />
