How was consumer input sought in development of your program plan and selection of <br />the services offered? Describe all activities undertaken in connection with this plan, <br />including consumer or public meetings; meetings with other agencies; presentations to <br />boards, commissions, or committees. If possible provide general dates for these activities. <br />(see questions 4A and 4B that follow) <br /> <br />Public input for the Annual Paratransit Plan was collected through the annual survey of riders and <br />the following public meetings: <br /> <br />February 2014 - Annual Survey of all Flex Shuttle Registered Riders <br />February 4, 2014 - Annual FLEX Focus Group Workshop <br />February 13, 2013 - Annual FLEX Focus Group Workshop <br />March 20, 2014 – The City of San Leandro Senior Commission <br /> <br /> <br />4A. If yes, list the committee name and date of the meeting. <br /> <br />The San Leandro Senior Commission will be reviewing our paratransit program plan, as well as the <br />survey results, at their next monthly meeting, which will be held on March 20, 2014. <br /> <br /> <br /> <br />5. Describe any surveys or analysis conducted to develop this plan and to select the range of <br />services your program offers? <br /> <br />Our annual paratransit survey was sent to all registered Flex Shuttle riders at the end of January. <br />This year we received a 22% response rate. We held our annual Focus Group workshops in <br />February. We also take comments and suggestions on a continual basis, either in writing or by <br />contacting the Paratransit Coordinator and suggestions are always discussed amongst staff. <br /> <br />February 4, 2014 - Annual FLEX Focus Group Workshop <br />February 13, 2013 - Annual FLEX Focus Group Workshop <br /> <br />Through the survey we gathered critical information such as: frequency of use, trip purpose, <br />service quality and future needs.
