<br />J.2. Application Requirements.Informational Required. Each applicant requesting an <br />Administrative Review conditionaluse permit under this Oordinance shall submit the <br />following: <br /> <br />1. Aa scaled site plan and a scaled elevation view and other supporting drawings, <br />calculations, and other documentation, signed and sealed by appropriate licensed <br />professionals, showing the location and dimensions of all improvements, including <br />information concerning topography, radio frequency coverage, tower height re- <br />quirements, dimensions of antennas, ancillary equipment and antenna sup- <br />port structures, setbacks, ingress and egress, drives, parking, fencing, land- <br />scaping, adjacent uses, and other information deemed by the Planning Division or <br />Board of Zoning Adjustments to be necessary to assess compliance with this Sec- <br />tion. <br /> <br />2. A completed Planning application. <br /> <br />3. A signed letter of authorization from the underlying property owner. <br /> <br />4. Photo-simulations of the proposed facility that identifies potential visual <br />impacts. Consideration should be given to views from nearby residential <br />areas, public rights-of-way, parks and open space, and other public <br />viewshed areas. <br /> <br />5. Manufacturer’s specification sheets for the proposed antennas and ancil- <br />lary equipment. <br /> <br />6. Photographs of the existing site conditions, as well as the surrounding <br />land uses. <br /> <br />7. A copy of the carrier’s FCC license. <br /> <br />8. A stamped and signed radio-frequency report prepared by a qualified engi- <br />neer. <br /> <br />9. A written description of the proposed project, including a description of the <br />type of proposed installation and the location and dimensions of the pro- <br />posed antennas, antenna support structures, and all ancillary equipment. <br /> <br />10. For treepoles, provide bark cladding and branch with leaf/needle sampes <br />for review and selection. <br /> <br />In addition to the requirements listed above, each applicant submitting a Condi- <br />tional Use Permit application under this Ordinance shall submit the following <br />additional application materials: <br />Exhibit G: Proposed Amended Article16 Page 62 <br />Note: underline and bolded text represents new text; strike-through text represents text to be eliminated
