I. A detailed written work plan, which shall include the following: <br /> a) Backup staffing plan to cover absenteeism, vacations, etc; <br /> b) The number and level of supervisors proposed; <br /> c) The type and quantity of equipment to be used per building; <br /> J. The Contractor shall provide a list of type and quantity of equipment that will be <br /> utilized at each location in the performance of this contract. A copy of the specification <br /> of each type of equipment shall be attached to the proposal. The submitted equipment <br /> shall be kept current and operating in all facilities at all times for the life of the contract. <br /> For general purpose vacuums and vacuums for low pile carpet, Contractor is <br /> encouraged to use vacuums approved by the Carpet and Rug Institute (CRI) Green <br /> Label Program to reduce building contaminants. For a list of certified vacuums, visit <br /> http://www.carpetrug.org/about-cri/cri-signature-prog rams/vacuum-list.cfm. <br /> (Items K, L and M are required within ten (10) working days from notice of <br /> award) <br /> K. City of San Leandro business license; <br /> L. Documentation (insurance certificates) showing compliance with the insurance <br /> requirements of the attached sample Non-Professional Services Agreement <br /> M. Enter into a Non-Professional Services Agreement (NPSA) with the City of San <br /> Leandro. <br /> TERMS OF CONTRACT <br /> The term of the Contract shall be from date of start of service by contractor until June 30, <br /> 2015; per section 1.1 of the NPSA. <br /> Option to extend for Good Performance: The City may, at its option, and with the <br /> approval of the Contractor, extend the term of the Contract for up to four (4) additional <br /> one-year terms, as deemed beneficial to the City, for a total contract period of five (5) <br /> years. Contractor shall be notified in writing of the City's intention to extend the Contract <br /> term at least sixty (60) calendar days prior to the expiration of the Contract. (see section <br /> 8.2 of NPSA, also) <br /> The Contractor may apply for a cost adjustment when notified in writing of the City's <br /> intention to extend the Contract term. In order for the rate adjustment to be considered by <br /> the City, the Contractor shall be in full compliance with all of the performance standards as <br /> established by this Contract. The rate may be adjusted by the Contractor as provided <br /> herein solely on the basis of the change in the Consumer Price Index for All Urban <br /> Consumers (CPI-U) for the San Francisco Bay Area, provided, however, that those <br /> adjustments in rates based solely on the change in CPI-U will not exceed five (5) percent <br /> per annum.
