EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Lake Chabot Road is a two-lane collector that runs east to west above Chabot Creek and Lake Chabot. <br />Approximately 2000 linear feet of roadway between Chabot Terrace and Astor Drive shows signs of <br />settlement due to movement of the hillside below the road. The road has several catch basins that <br />discharge to the hill side between the road and creek. Visible erosion has occurred at some of the storm <br />drain discharge points. This project includes design, permitting, and construction of hillside stabilization as <br />well as improvements to the channelization of water into existing catch basins and improvements to the <br />storm drain out falls that these drains are connected to. A preliminary design has been prepared as part of <br />a geotechnical investigation in 2011. <br /> <br />This contract is for design of the abovementioned improvements as detailed below. <br /> <br />Task 1 Project Management and Kick off Meeting <br /> <br />Consultant will coordinate with the City and orient Consultant personnel with the current site <br />conditions, preliminary design, potential site constraints, and precise needs of City staff. This will <br />be accomplished by holding a kickoff meeting with the City. Through a kickoff meeting the <br />proposed scheduled and milestones will be reviewed. At the kickoff meeting, Consultant will <br />review, with City Staff the background of the project, including site history, topographic survey, <br />and identification of project constraints such as environmental issues, property ownership, and <br />the like. Parties will discuss how the City selected the stabilization method identified in the RFP <br />and discuss the opportunities for value engineering to reduce the construction cost of the <br />project. <br /> <br />Task 1 Deliverables <br /> <br />• Meeting Minutes/Summary notes from kickoff meeting, <br />• Project Schedule <br /> <br />Task 2 Topographic Survey <br /> <br />This task will include site review, right of way surveys and research, topographic surveys, drafting, <br />and utility mapping. <br /> <br />Site Review: A topographic survey will be prepared by Consultant which will include locations of <br />existing improvements, utilities, critical items which may affect project schedule and exiting survey <br />control. Consultant will take detailed photos and notes of the critical items on Lake Chabot Road. <br /> <br />Right of Way Survey and Research: Title reports and record maps from the City will be obtained by <br />Consultant. Consultant will locate and tie in existing property corners and monuments found in the <br />field, and determine the existing right of way (ROW) and property lines from title, record map, and <br />1089
