- 14 - <br />CDBG Agreement with Child Abuse Listening, Interviewing and Coordination Center (CALICO) FY2019-2020 <br />EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2019 – June 30, 2020 <br /> <br />Agency: Child Abuse Listening, Interviewing and Coordination Center (CALICO) <br />Address: 524 Estudillo Avenue <br /> San Leandro, CA 94577 <br /> <br />Contact Person: Erin Harper, Executive Director <br /> <br />Phone: 510 895-0702 <br /> <br />Project Name: San Leandro Child Abuse Intervention Project <br /> <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />CALICO, the Child Abuse Listening, Interviewing and Coordination Center, will provide family support services <br />to improve mental health outcomes for San Leandro toddlers, children and adolescents and adults living with <br />developmental disabilities who have suffered physical or sexual abuse or neglect, and the caregivers of those <br />victims. <br />MAJOR ACTIVITIES <br /> <br />A CALICO Family Resource Specialist (a skilled, mental health clinicians) will provide on-site crisis intervention, <br />information, referrals and support and follow-up services by phone to caregivers to enhance their mental health <br />outcomes and ability to provide support and foster the positive mental health of their traumatized child. <br /> <br />PERFORMANCE MEASURES <br /> <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT: <br /> <br />1. A Family Advocate (FA) will conduct an on-site crisis assessment with at least 75% of San Leandro <br />families at CALICO to assess immediate safety needs and provide appropriate crisis intervention and <br />stabilization. <br /> <br />2. An FA will initiate a California Victim Compensation Program (CalVCP) application (or ensure one was <br />already completed) on behalf of at least 50% of caregivers (to access funds for mental health services for <br />the caregiver). <br /> <br />3. An FA will initiate a CalVCP application (or ensure one was already completed) on behalf of at least 75% of <br />child-victims (to access funds for mental health services for the child). <br /> <br />4. An FA will provide psycho-education to at least 75% of caregivers to increase their understanding of the <br />impact of trauma on children and appropriate parental response. <br /> <br />5. An FA will contact at least 70% of caregivers by phone post-interview to providing ongoing intervention, <br />information and support. <br /> <br />6. At least 50% of caregivers contacted by telephone will report that their child is enrolled in counseling. <br /> <br />7. At least 75% of caregivers will indicate a favorable response to their child’s treatment at CALICO. <br />93
