- 14 - <br />CDBG Agreement with Spectrum Community Services FY2019-2020 <br />EXHIBIT A <br /> <br />SCOPE OF WORK <br /> <br />Term of Contract: July 1, 2019 – June 30, 2020 <br /> <br />Agency: Spectrum Community Services <br /> <br />Address: 2621 Barrington Court <br /> Hayward, CA 94545 <br /> <br />Contact Person: Lara Calvert, Executive Director <br /> <br />Phone: 510 881-0300 <br /> <br />Project Name: San Leandro Senior Nutrition Meals Program <br /> <br />PROJECT GOALS AND OBJECTIVES <br /> <br />Spectrum Community Services will serve 210 low-income San Leandro seniors—3,675 hot, nutritious meals per <br />year at San Leandro Senior Center and Fargo Senior Apartments. <br /> <br />MAJOR ACTIVITIES <br /> <br />The Senior Nutrition Meals Program will serve lunches five (5) days a week, up to 248 days per year, at the <br />San Leandro Senior Community Center. Each day freshly prepared, nutritious lunch-time meals will be <br />prepared at Spectrum Community Services’ central kitchen located at Josephine Lum Lodge in Hayward. The <br />meals are carefully planned and prepared to be delicious, low salt, and nutritionally balanced to supply 1/3rd of <br />the U.S. Recommended Daily Allowance for older adults. After preparation, meals will be packed in bulk <br />containers and delivered by Spectrum’s delivery driver to the San Leandro Senior Center. <br /> <br />A Spectrum Site Coordinator and a Meal Handler work with Senior Nutrition Program staff, San Leandro Senior <br />Center staff, and volunteers to serve the meals. The Senior Nutrition Program staff or volunteers welcome <br />each participant for lunch, get them signed-in, register new participants, and receive voluntary donations. No <br />senior is ever turned away for inability to pay the suggested donation. To protect participants from any <br />embarrassment of not being able to donate, all donations are anonymous, not tracked by person. Spectrum <br />maintains records of donation amounts received at the lunch site, not by participant. Current year average <br />donation at the SLSC is $1.83 per meal while the suggested donation is $3.75. <br /> <br />As part of Spectrum’s Senior Nutrition Program, staff make quarterly presentations on healthy living and <br />nutrition and take questions from program participants. Spectrum includes nutrition information and tips on the <br />flip side of the lunch menus, and staff and volunteers are trained to reinforce connections between nutrition, <br />physical activity, and health. <br />PERFORMANCE MEASURES <br /> <br />SUBRECIPIENT SHALL, FOR THE TERM OF THE CONTRACT: <br /> <br />Performance will be measured by the number of meals served and the number of San Leandro seniors who <br />participate in the program, with data available on a monthly, quarterly and annual basis. During each year of <br />the two-year grant, Spectrum will serve at least 3,675 nutritious meals to at least 210 San Leandro seniors. <br /> <br />Success for this program is defined not only by meeting or exceeding our goals and objectives, but by the <br />quality of the services provided.
