<br />Consulting Services Agreement between City of San Leandro and Last revised June 26, 2020 <br />Ethosoft, Inc. for Laboratory Information Man agement System Exhibit A – Page 1 of 10 <br /> <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Introduction <br /> <br />This Scope of Services defines the scope of work to be accomplished by Ethosoft under the terms and <br />conditions of the contract for the Laboratory Information Management System (LIMS) Project between the <br />City of San Leandro Water Pollution Control Plant Laboratory (referred to herein as “the City”) and Ethosoft, <br />Inc. The tasks to be performed by Ethosoft are defined (see Section B, Ethosoft Responsibilities) as are <br />the activities to be performed by the City (see Section C, San Leandro Responsibilities). If major changes <br />to this Scope of Services are identified, they will be processed in accordance with the plan described in <br />Section D, "Project Change Control Plan". The analysis and implementation of changes may result in <br />modifications to the Estimated Schedule, additional charge s to the City, or other terms of this Scope of <br />Services. <br /> <br />The Scope of Services includes the following subsections: <br /> <br />A. Scope of Services <br /> <br />The scope of the project is to establish a fully functional Cloud X -LIMS system that will be implemented for <br />the laboratory to provide a comprehensive solution that will meet the requirements of the City. This system <br />will be used by the City’s laboratory technicians and managers to perform sample analysis, verification, track <br />lab supplies and provide reporting. <br /> <br />Phased Approach <br />A detailed description of Ethosoft’s 7-phased implementation process is outlined below: <br />Phase I: Project Organization -- The first phase is to get the project organized. During this phase, the project <br />scope is reviewed with the City’s project manager and there is a project kick off meeting to spell out roles, <br />responsibilities, and lines of communication during the project.
