<br />Consulting Services Agreement between City of San Leandro and Last revised 03/10/2022 <br />Building Futures with Women and Children for Mobile Street Outreach Exhibit A – Page 4 of 4 <br /> Attend regular meetings; <br /> Serve on workgroups and subcommittees as needed; <br />Administer follow-up surveys to help improve service delivery and program effectiveness of the City of <br />San Leandro's Coordinated Outreach program. <br /> Ensure timely submission of monthly, annual, and other reports as requested by the City of San <br />Leandro. <br /> Submit monthly reports by the 10th of the month for services provided the prior month. <br /> Submit a final narrative report 15 days after the completion of the contract. <br /> <br />III. Program Evaluation and Reporting Requirements <br /> As a provider of services or housing to homeless and at-risk households in San Leandro, <br />Consultant shall participate in the system-wide Initiative to Measure Success and Report <br />Outcomes. <br /> Consultant shall collect required information on clients and services provided and enter data into <br />the Alameda County HMIS system or, if exempted from participation, training and technical <br />assistance will be provided to utilize the system. <br /> At minimum, Consultant shall submit in its monthly report the following metrics: <br />o Number of unsheltered clients served as extracted in HMIS. <br />o Number of new, unsheltered individuals entered in HMIS during the reporting period. <br />o Description of services provided and number of people receiving services. <br />o Changes in the number of unsheltered individuals engaged weekly with local service <br />providers. <br /> Consultant shall use boilerplate reports from the County HMIS system to track and report on <br />outcomes adopted for the Initiative. These boilerplate reports will be modified from time to time as <br />outcomes expectations or reporting requirements are adjusted. <br /> Consultant shall receive periodic system-level and collaboration processes, which may include, <br />but are not limited to: <br />o Homestretch/Coordinated Entry Processes <br />o Collaborative meetings with the City San Leandro other community partners as requested <br />by City staff <br /> <br /> <br />
