The term of this SOW will commence on the SOW Effective Date and shall continue through <br /> (“SOW Term”). <br />3.2 Milestones. <br /><This section outlines the timeframe for completion of project milestones or tasks described in the <br />approach, where appropriate. Sometimes, usually when the SOW Effective Date is uncertain (i.e., “upon <br />second signature”), it may be useful to state the milestone dates as calendar or business days in relation to <br />the SOW Effective Date (e.g., if SOW Effective Date is Day 0, Milestone 1 might be described as being due <br />on Day 45, or “45 days from SOW Effective Date”). If there are no Milestones, this subsection may be <br />deleted.> <br />4. DELIVERABLES <br /><In this section, you should list the items that should be produced by VIP and delivered to City as a result <br />of this project. At a minimum, a list must be included, but descriptions of each deliverable, including format <br />(e.g., in Microsoft Word, PowerPoint), manner of submission (e.g., paper, electronic) and acceptance <br />criteria/process (e.g., “City shall have 5 business days from receipt to approve/disapprove of each <br />Deliverable submitted by VIP…”) are preferred.> <br />NOTE – the below table is a suggested format, and should be tailored to the requirements of the scope of <br />work or to best present the requirements. For example, a table format may be useful and may include <br />information beyond the below. <br /> <br />In connection with VIP’s performance of the Services, VIP shall deliver the following deliverables to City by <br />the due dates and in the formats listed and described below (the “Deliverables”): <Always use preceding <br />language prior to a table or a list of certain Deliverables> <br /> <br /> Deliverable (and/or Phase) Format / Description / Acceptance Criteria (if <br />not discussed in separate section) <br />Due Date <br />4.1 Kickoff Meeting Minutes (Phase <br />1) <br />• Describe kick-off meeting attendees, <br />discussion, and decision <br />• Include first version of Risks, Agreements, <br />Issues, and Decisions (RAID) document for use <br />throughout the project <br />• Submitted in Microsoft Word format <br />• City to accept/reject within 5 business days of <br />receipt <br /> <br />May 1, <br />20XX <br />4.2 Analyze utilization data for <br />diabetes patients to determine <br />xxxxx <br />• Analysis method complies with City <br />requirements as stated in Attachment X <br />• Report – submitted electronically in Microsoft <br />Word format <br />• City to accept/reject within 5 business days of <br />receipt <br />June 1, <br />20xx <br />453
