I . Pretax payments of employee contribution toward premium; <br />2. Medical Care Flexible Spending Account up to a $1,000 maximum per benefit plan year; <br />3. Dependent Care Flexible Spending Account up to a $5,000 maximum per benefit plan year; <br />4. A "Core Flex" medical plan; <br />5. A payment of One Hundred Fifty and no/100 Dollars ($150.00) per month for employees <br />electing not to enroll in the medical and dental plans OR a payment of One Hundred and <br />no/100 Dollars ($100.00) per month for electing not to enroll in the medical plan only. <br />Effective March 1, 1996, the City will contribute the following monthly amounts toward the election <br />of benefits in the plan: <br />Employee $216.48 per month <br />Employee Plus One 368.83 per month <br />Employee Plus Family 469.40 per month <br />Employees electing to enroll in the Health Net or Blue Cross Prudent Buyer Plan will be required <br />to contribute the difference between cost of the premium for the Kaiser Plan and the actual cost of <br />the medical plan premium. Such employee contribution will be on a pre-tax basis as specified <br />above. <br />Effective August 1, 1996, these amount will be adjusted by an amount equal to the change (increase <br />or decrease) in the Kaiser Plan ($5 copayment). <br />B. Dental Plan <br />1. San Leandro Police Officers' Association <br />For those eligible employees represented by the San Leandro Police Officers' Association and their <br />eligible dependents, effective Tare„x9+, the City shall pay the amount for dental plan coverage <br />not to exceed a maximum of Sixty-eight Dollars and Fourteen Cents ($68.14) per month for current <br />an <br />dental coverage provided by1 etfe Insurance Company. Y q An subsequent increases in <br />dental insurance premiums shall be split equally between the City and the employee. <br />2. San Leandro Management Organization <br />For those eligible employees represented by the San Leandro Management Organization and their <br />eligible dependents, effective July 1, 1995, the City shall pay the amount for dental plan coverage <br />not to exceed a maximum of Sixty-eight Dollars and Fourteen Cents ($68.14) per month for current <br />dental coverage provided by MetLife Insurance Company. Any subsequent increases in dental <br />insurance premiums shall be split equally between the City and the employee. <br />23 <br />
