(Header for Odd Numbered Pages: Times 8 point, bold, flush right) Action Plan - FYI 997 <br />City of <br />Page 7 <br />White Title II CARE Consortium, the AIDS Advisory Board, The HOME Technical Advisory <br />Committee, the Urban County Technical Advisory Committee, the Southern Alameda County <br />HIV Task Force, the Emergency Services Network, East Bay Housing Organizations and the <br />Housing and Community Development Citizens' Advisory Committee. The Plan was adopted by <br />the Board of Supervisors on April 9, 1996. <br />In FY95, $945,711 in HOPWA funds were made available to cover five priority areas for the <br />plan. These areas were: Housing Information and Referral; Transitional Housing; Permanent <br />Housing Set -Asides; Planning for a Long-term Care Facility; and Technical Assistance. To date <br />commitments have been made, and work has begun, on the development and operation of an <br />AIDS Housing information hotline (operating out of Hayward), development of transitional <br />housing for people with AIDS and mental illness in Oakland of which no fewer than six beds will <br />be dedicated to people with AIDS, and planning for a licensed long-term care facility or facilities. <br />Negotiations are underway for as many as 28 units of permanent housing set -aside for families in <br />Oakland, Alameda and Livermore, and for technical assistance to three Oakland -based AIDS <br />housing providers. <br />In addition to activities funded with FY95 HOPWA funds, several other aspects of the plan have <br />been carried out using other funding sources, or at no cost. These include the start up of PROJECT <br />INDEPENDENCE, a HOPWA Special Project of National Significance funded directly by HUD to <br />provide shallow rental assistance and accessibility modifications to people living with HIV/AIDS <br />throughout Alameda County. Three agencies are administering the rental assistance and support <br />service component of this program: The Ark of Refuge and AIDS Project of the East Bay in <br />Oakland and the Tri-City Health Center in Fremont. Alameda County also received funding in <br />1996 for additional Shelter Plus Care certificates to be targeted specifically to homeless persons <br />with AIDS and related disorders. This program is expected to begin operation in Spring 1997. <br />HCD also completed draft standards of care for AIDS housing to be used in both counties of the <br />Eligible Metropolitan Area. <br />Alameda County HCD has just entered into a contract with the City of Oakland for $1,055,528 in <br />FY1996 HOPWA funds. These funds will be used to meet other priorities set forth in the AIDS <br />Housing Plan, including supporting the operation of service -enriched emergency shelter for <br />homeless people with AIDS; the development of permanent, supportive housing for dually and <br />triply diagnosed persons; and continued support for family units set -aside; long-term care planning <br />and technical assistance. <br />- 6u <br />
