C. Life Insurance <br />1. San Leandro Management Organization <br />The City shall maintain in effect employer provided Term Life Insurance in the amount of Fifty <br />Thousand ($50,000) Dollars with an AD&D benefit. <br />2. San Leandro Citmployees' Association <br />The City shall maintain in effect employer paid Term Life Insurance in the amount of Twenty <br />Thousand ($20,000) Dollars with an AD&D benefit. <br />D. Long Term Disability <br />1. San Leandro Management Organization <br />The City shall maintain in effect an employer paid Long Term Disability Insurance Program with a <br />benefit percentage of 40% of base monthly earnings with a maximum gross monthly benefit of <br />$5, 000. <br />2. San Leandro Citmployees' Association <br />The City shall maintain in effect an employer paid Long Term Disability Insurance Program with a <br />benefit percentage of 40% of base monthly earnings with a maximum gross monthly benefit of $5,000. <br />Section 2.2 RETIRED SALARIED OFFICERS AND EMPLOYEES <br />The provisions of Subsections A through D below apply to an eligible retired City employee and not more than <br />one (1) dependent. Retired employee dependent eligibility for City medical plan contribution is conditional upon <br />the active enrollment of the retired employee. <br />For those retired salaried officers and employees who are currently members of one of the City's three medical <br />plans, the City shall pay medical plans' two-party rate costs until the member is eligible for Medicare coverage as <br />follows: <br />A. San Leandro Police Officers' Association <br />This Subsection A applies only to those retired City employees who were assigned to classifications <br />represented by the San Leandro Police Officer's Association. Effective January 1, 1998, the City shall <br />contribute to the health plan two-party rate costs until the retired employee is eligible for Medicare coverage. <br />The maximum amount to be contributed by the City is as follows: <br />22 <br />
