Action Plan - FY1998 <br />City of San Leandro <br />Page 9 <br />supportive housing for dually and triply diagnosed persons; and continued support for <br />family units set -aside; long-term care planning and technical assistance. <br />Shelter Plus Care <br />Alameda County HCD is the grantee for four competitive grants under HUD's Shelter Plus <br />Care Program, which provides housing and supportive services on a long-term basis for <br />homeless persons with disabilities. HCD jointly administers the program with the <br />Alameda County Health Care Services Agency, the Oakland Housing Authority, and the <br />Housing Authority of Alameda County. The program benefits primarily those individuals <br />with serious mental illness, chronic problems with alcohol and/or drugs, and/or acquired <br />immunodeficiency syndrome (AIDS) or related diseases who are living in places not <br />intended for human habitation (e.g. on the streets or in shelters). <br />Shelter Plus Care provides assistance to clients through rental assistance payments and the <br />provision of a range of supportive services which are coordinated by over 35 community - <br />based organizations. <br />Shelter Plus Care Program grants are made by HUD on a competitive basis to local <br />jurisdictions and agencies. HCD currently administers a five-year Sponsor -based rental <br />assistance (SRA) grant and a ten-year Single Room Occupancy Assistance (SRO) grant; <br />together these amount to $9.2 million. The SRA grant goes towards 74 units located <br />throughout the County and the SRO grant goes towards 92 units in Oakland. HCD also <br />administers a five-year Tenant -based rental assistance (TRA) grant of $9.8 million to <br />provide assistance to 214 units. Approximately half of the TRA grant, $4.9 million, is <br />being spent to assist Shelter Plus Care participants living in the HOME Consortium. HCD <br />was recently awarded an FY96 TRA grant of $1.47 million specifically to provide <br />housing assistance to Shelter Plus Care eligible persons living with AIDS. This grant is <br />being jointly administered with the City of Berkeley, with 15 certificates going to <br />Berkeley, and 20 to cover the City of Oakland and the HOME Consortium. Start-up is <br />expected in early 1997. <br />Alameda County Housing/Jobs Linkages Program (Supportive Housing Program) <br />In FY95, Alameda County HCD received a three year Supportive Housing Program grant <br />from HUD for $2.5 million for the Alameda County Housing/Jobs Linkages Program. <br />This grant proposal was a joint effort of HCD and the Alameda County Associated <br />Community Action Program (ACAP). The program provides transitional rental assistance, <br />job development and support services for homeless families connected with one of seven <br />shelters in the HOME Consortium.
