4. Obtain quarterly, or more frequently if appropriate, itemized billings from outside <br />adjusters/investigators and legal counsel for payment consideration. <br />5. Report as appropriate to the City and the excess workers' compensation insurance carrier <br />necessary information on the current status of claims as required. <br />6. Reconciliation of bills paid on a frequency determined by the City. <br />7. Prepare on behalf of the City, the Public Self -Insurers' Annual Report in accordance with <br />current State Requirements. Completion of this report is required to be submitted to the City <br />no later than thirty (30) days prior to the due date. <br />1. Payments to employees, providers and vendors will be made by the Claims Administrator. <br />2. Payments to employees, providers and vendors must be made from the City's trust account. <br />3. A monthly check register must be provided within five (5) working days after the month end <br />indicating all checks issued during the month. The register must be on a calendar -month <br />basis. <br />Communications: <br />1. Open communications will exist and be maintained bilaterally between the City and the <br />Claims Administrator. Phone calls will be returned promptly. <br />2. The Claims Administrator will provide the City with notices dealing with changes or <br />proposed changes in Workers' Compensation administrative procedures and laws. <br />3. The Claims Administrator will meet bi-weekly with the City to evaluate open indemnity <br />claims including litigated cases. Additionally, a quarterly meeting will be held to outline <br />losses and identify problems, examine reserves identify current trends, discuss relevant <br />changes in the California State Labor Code, apply new case law to existing claims, discuss <br />the need to improving or altering claims management, to make recommendations for <br />improvement in communications between the Administrator and to the City. <br />City of San Leandro <br />Standard Contractual Services Agreement <br />5/29/97 <br />AGRE\STANDARD.DOC <br />251 <br />
