EXHIBIT B <br />PAYMENT SCHEDULE <br />City shall pay Claims Administrator for services to be performed and reimbursable costs incurred <br />pursuant to this Agreement. Claims Administrator shall submit invoices, not more often than <br />once a month during the term of this Agreement, based on the cost for services performed and <br />reimbursable costs incurred prior to the invoice date. Invoices shall contain the following <br />information: <br />1. Serial identifications of progress bills, i.e., Progress Bill No. 1 <br />2. The beginning and ending dates of the billing period. <br />City shall make monthly payments, based on such invoices, for services satisfactorily performed, <br />and for authorized reimbursable costs incurred. <br />City shall pay the total sum due pursuant to this Agreement within forty-five (45) days after <br />completion of the services and submittal to City, if all services due pursuant to this Agreement <br />have been satisfactorily performed. <br />Cost of Claims Administrator Services will be as follows: <br />1. Annual Claims Administration Fee: $3,600.00 <br />This includes the following services: Computer input of all open claims, monthly <br />computer loss information and quarterly and annual reports, trust account (excluding <br />checks and bank charges), and management attendance at claim review meetings. <br />2. Management of all workers' compensation claims, including, but not limited to, <br />adjusting, inside investigations and claims management services: $6,050.00 per month, <br />up to 103 incoming files. Files in excess charged: $65.00 per month. <br />3. Outside workers' compensation investigations may be done after authorization by <br />the City at the following time and expense rates: <br />a) Investigation Services $46.00 per hour <br />b) Office Overhead 33% of services <br />c) Secretarial $ 5.00 per page/form <br />City of San Leandro <br />Standard Contractual Services Agreement <br />5/29/97 <br />AGREWANDARD.DOC <br />- 253 <br />
